Chart Note | Change Provider

The provider associated with the note can be changed for Office Visits, Phone Calls, Lab Visits, and Letters. You can also set a supervising provider on the note from the same view.

Note: The provider and supervising provider can only be updated while the note is in an unlocked state. If the note is locked/signed, unlock/amend it before making changes.

Step by Step

Begin with the desired patient's chart open.

  • Navigate to the desired note.

  • Click on the triple dot located at the top right of the note.

  • Select Change Provider from the drop-down menu. The header will change to allow you to update the note's provider.

  • Type to search for people and/or scroll and select from the dropdown.

  • Select Save.

TIP: If the update needs to be cancelled, select Cancel at any time.

Setting a supervising provider

When the header is in edit mode, you'll also see a Supervising provider field alongside the Provider field.

Use this to record the physician supervising the encounter — for example, when a visit performed by an NP or PA will be billed under a supervising physician.

  1. With the note header in edit mode (via Change Provider above), open the Supervising provider dropdown.

  2. Type to search, or scroll and select the supervising provider.

  3. Select Save.

Once saved, the supervising provider appears in the note header alongside the provider, for example:

On Tuesday, 6/16/26 at 5:46 PM EDT with Canvas Support, AD, MD (supervised by Allison Doctor, MD) at Canvas Los Angeles Office

What this does: The supervising provider you set here carries onto the claim when it's created from the note. It does not by itself bill the claim incident-to — incident-to is turned on separately on the claim. See 📄 Claim Management for billing a claim incident-to.

Note: A supervising provider must have a first and last name, a valid NPI, and a phone and work address on their profile — the same requirements as for prescribing. Incident-to claims can't be submitted if the supervising provider is missing a valid NPI.
