Canvas Patient Safety Scale

Introduction
Patient safety issues are those that interfere with the process of health care and could lead to diagnostic errors, medical errors, injury, or other preventable harm to a patient during the process of health care (AHRQ).
This article defines the patient safety scale we utilize to assess and prioritize events with high clinical severity that fall in the realm of patient safety. We understand how critical patient safety concerns are and it is our top priority to address them as quickly as possible and ensure we perform the necessary validation to prevent further occurrences.
Scale
P1 Patient Safety
Patient safety issue with low likelihood of human mitigation.
Canvas users are unlikely to notice the issue and/or they are not reasonably able to take steps to prevent patient harm.
If either of these is true, there is a low likelihood of human mitigation.
Example- Canvas fails to associate a medication with the appropriate code and sends
the wrong medicine to the pharmacy.
P2 Patient Safety
Patient safety issue with high likelihood of human mitigation.
Canvas users are likely to notice the issue and they are reasonably able to take steps to prevent patient harm.
If both of these are true, there is a high likelihood of human mitigation.
Example- Prescribe command fails for all patients, with an error message in the UI.
