The Structured Assessment command allows templated assessments for common conditions enabling ICD-10 and CPT at the assessment level. Creating Structured Assessments follows the same workflow as creating a new questionnaire. A Structured Assessment can be leveraged to easily diagnose and assess a condition in Canvas. ICD-10 and CPT codes can be linked to answers within the Structured Assessment. When recording the Structured Assessment, if any of the selected the answers are linked to ICD-10 or CPT codes then these codes will drop into the billing footer and also update the conditions list in the patient summary.
Q: What is the difference between a questionnaire and a structured assessment?
A: Although structured assessments are are created using the same workflow as creating a new questionnaire. Some of the differences include the following:
Structured Assessment is sorted into the assessment part of a note and a questionnaire is sorted into the subjective part of the note.
You will enter "SA" instead of "QUES" into the cell located in column C corresponding to the field titled "use_case_in_charting".
The naming convention for the tab containing the structured questionnaire within the Loader Spreadsheet is also different. Instead of beginning with "ques-" the tab containing a Structured Assessments would have to begin "sa-".
Q: What are the rules for assigning codes to answers and questions?
A: Codes for answers within a question MUST be unique. However, questions can share codes since they are assigned different IDs.

Follow the instructions for Creating a New Questionnaire
Ensure that you've entered "SA" in the use_case_in_charting field (Column C)
Ensure you've renamed the Copy of TEMPLATE tab with the "sa-" naming convention.
Ensure that there is no "." in the ICD10 codes you are using.
Begin in an open note
Type "Structured Assessment"
Select the desired Structured Assessment from the drop down menu in the "Structured Assessment:" field
Fill out the Structured Assessment as needed
Select Record to commit the command
Ensure that the appropriate ICD-10 and/or CPT codes have dropped into the billing footer and that the conditions list has updated
NOTE: The structured assessment will show up in the assessment part of the note when printed.
