Appointments | Unstructured Reason for Visit

The Reason for Visit command frames the motivation for the documented encounter. Here, the patient's "chief complaint" and "reason for visit" are used interchangeably. 

Step by Step:

This process begins in an open note of the patient chart.

  • Type "reason for visit" "cc" or "rfv" into a blank line within the note.

  • Select the command by pressing Enter on the keyboard.

  • With the command selected, the cursor will default to free text.

  • In free text, type the reason for the visit.

    TIP: Use Enter on the keyboard to roll the input cursor to a new line.

  • The Reason for Visit populates in the heading of the current note.

Reason for Visit in Appointment Scheduling  

  • Navigate to the desired date of appointment on the Schedule.

  • Click and drag on the desired time slot as shown below. 

  • A new appointment card appears. 

  • Navigate through and fill out the fields as follows:

    • Link the patient: type patient name into the field 

    • Select the type of visit.

    • Reason for Visit: type the reason for visit in the provided text box

  • Click Book Now.

Reason for Visit in the Follow Up Command

  • When documenting a follow-up, a Reason for Visit field is available to document the reason for the follow-up appointment.

  • With the command committed, a new appointment note is created for further scheduling. 

  • The Reason for Visit command is automatically filled and populated within the body of the appointment note.
