The Family History command allows the user to document the patient's family history. 

FAQ:

Q: Can I add multiple family members to a specific condition?

A: Yes - you will enter multiple family history commands for each member with the same condition.

 

Q: Can I enter multiple conditions for the same family member?

A: Yes - you will enter multiple family history commands for each condition for an individual family member.

 

Q: How do I document the patient is adopted?

A: Enter "not known - adopted" as the condition and the relative as "adopted"

Q: I made a mistake, how do I remove this command?

A: Commands can be entered in error, but not deleted.  Please review the Entering a Command in Error document for directions.  

Step by Step

This process begins in an open line of a note of the patient's chart. 

  • Type "family", "history", or "fhx" into a blank line within the note.

  • Select command by hitting Enter on your keyboard.

  • Type the condition of the family member.

  • Hit Enter on the keyboard once the desired condition populates.

  • Navigate through the remaining fields by selecting Tab between each field

    • Relative: the family member's relationship to the patient

    • Note: any additional notes or comments such as the age of diagnosis, or any other relevant information. Field has a 512 character limit.

  • Click RECORD to commit the command to the note. 

    NOTE: The entry immediately becomes visible in the left side panel under Family History
